If you want to make an appointment with one of the many behavioral health professionals at St. Louis Behavioral Medicine Institute or enroll in one of our Intensive Outpatient or Partial Hospitalization Programs, our Intake Team would be glad to assist you.

We accept most major insurance companies, but we do not accept Medicaid Primary and Ambetter.

Please choose from one of the following methods to request a new patient appointment:

• Complete our online appointment request form.
• Call 314-289-9411 to speak with an Intake Team member. Please be aware that hold times can be long (1-2 hours) so we recommend you leave us a voicemail for a return call. If you leave a voicemail, please include your name and your phone number. Our Intake Department cannot call you back without this information.
• Email [email protected] to request a return call from our Intake Team member. Please make sure you include your name and phone number.

TIPS

Our Intake Department will call or email you back within a few business days with additional information.

To respect your privacy, the Intake Department will attempt to call or email you back one time but they will not continue to try to reach you without a response.
