We want to first thank you for allowing our team to provide you or your loved one treatment! We know how important receiving high quality behavioral health treatment is and we want every patient and visitor at St. Louis Behavioral Medicine Institute to have a positive experience.

How to Make a Request

If you would like to request additional treatment or services in addition to the care you already receive, our Intake Team would be glad to assist you. You can request additional services by one of the following methods:

  • Complete our online appointment request form.
  • Call 314-289-9411 to speak with an Intake Team member. Please be aware that hold times can be long (1-2 hours) so we recommend you leave us a voicemail for a return call. If you leave a voicemail, please include your name and your phone number. Our Intake Department cannot call you back without this information.
  • Email [email protected] to request a return call from our Intake Team member. Please make sure you include your name and phone number.

Please be aware

  • Our Intake Department will call or email you back within a few business days with additional information.
  • To respect your privacy, the Intake Department will attempt to call or email you back one time but they will not continue to try to reach you without a response.
  • If you do not have a voicemail set up on your phone, we recommend providing us with another method of contacting you.

How to Request a Change in Provider

You have the right to request a change in provider. Change of Provider requests are reviewed and completed through an approval process. If your request is not approved, you will be notified as to why. Sometimes, we may not have a provider who fits the needs you are requesting or wait times for the provider you are requesting can be long. If that happens, we will share with you alternative options, which could include external referrals to agencies outside of St. Louis Behavioral Medicine Institute.

To request a change in provider, please contact our Front Desk team for the form. Our Front Desk team can be reached at 314-534-0200 (press option 2) or 636-532-9188 (press option 2). Call volumes can be high so please plan on leaving a voicemail with your name, phone number and callback as well as reason for calling and our Front Desk Team member will return your call. You can also email our Front Desk team to request the form at [email protected].
