Request Medical Records
You have the right to review and obtain a copy of your Protected Health Information contained within St. Louis Behavioral Medicine Institute’s Designated Record Set, except in limited situations as described in your Patient Handbook.
All record requests are processed in the order they are received and may take up to 30 days to process.
If you need to request your Protected Health Information for yourself or to send to someone else (like another provider or facility), please contact us for more information on submitting your request. Please email [email protected] to start the medical record request process.
